I developed a strong desire to help people, which led to obtaining an ADN in 2007 and a BSN in 2021. I am currently working towards an MSN with a focus in Nursing Education.

After obtaining my master's degree from Minot State University, I transitioned into nursing, because I truly wanted to be a part of the positive change in people's lives. I am particularly passionate about mental health wellness and optimizing patient healthcare outcomes through collaborative efforts with a multidisciplinary team. Transitioning from an inpatient setting to a care coordination role—which involves addressing the needs of a diverse population and the idea of bringing resources together for those who need it—most resonates with me the most.

I am committed to identifying risk factors that can affect patient safety and overall health quality, as well as connecting patients to available community resources based on their needs. My goal is to contribute to enhancing the well-being of individuals and ensuring they receive the appropriate support for their healthcare journeys.

I was in my last position for eleven years at NC Specialty Hospital in their preop department where I served as charge nurse and preceptor until accepting this new opportunity with PHMO.

I began working in community behavioral health in 2007, obtained my Master’s in Social Work in 2013, and received my clinical license in 2015. I have had a variety of roles including clinical, administrative, and leadership, and through it all, my one constant has been my love for helping others to meet their goals and achieve holistic health. I am passionate about the work we are doing here at PHMO to help our patients achieve their health goals and overcome the barriers they face.

I returned to NC and completed almost 19 years at WakeMed Cary Hospital, which included four years as a staff nurse, four years as a clinical care coordinator for the Hospital Medicine Service, and eleven years as an inpatient case manager. I am returning to my home at Duke Hospital as a care manager in the Resource Center.

Coming from Duke Raleigh Hospital Case Management Department, where I worked for the last 4 years, I have a special passion for Complex Case Management. I found it rewarding and enjoyed assisting patients with the safe discharges from the hospital. In particular, I enjoyed figuring out the reasons and barriers which prompted each patient to keep coming back to the ED or hospital, and then resolve them. I believe that I will be able to continue providing excellent services for the Population Health here at the Resource Center.

I have lived in North Carolina since 1995. I obtained my BS in Human Resources from Concordia University in Ann Arbor, Michigan and my MS in Health Promotion from Nebraska Methodist College in Omaha, Nebraska. I was a collegiate athlete and love all types of sports. My career has spanned the healthcare field for over 25 years in fitness, wellness, health coaching, and health promotion. I love helping others become their best selves. I am a creative in paint and fiber arts and teach art classes in Chatham County. I am also a volunteer proofreader for a large women’s organization out of Charlotte. I believe we become whole by addressing our spiritual, physical, emotional, intellectual, social, vocational, financial, and environmental needs.

I worked ten years in the school system before completing my bachelor’s and master’s degrees in social work. The vast part of my social work experience comes from the field of child welfare (with a focus in families and children) with Durham County Department of Social Services for over six years as an In-Home Service and Foster Care social work case manager. I later transitioned to social work and medical case management via Duke University Hospital. I have always had a passion for supporting people in the community by way of educating them and providing them with the necessary tools that would enable them to be their best selves. I believe social work is “heart work,” and it often begins by extending grace and lending a hand to some of our most vulnerable populations so that they have an opportunity to thrive and live their most optimal lives.

I have recently completed a Master’s Degree in Public Health Nutrition at Appalachian State University. Prior to joining DukeWELL, I spent six years with NC State University, providing nutrition education programs and leading health initiatives to youth and adults across the Triad. I enjoy building relationships with patients and helping them break down complex needs into small, tangible steps towards their healthcare goals! In my free time, I enjoy cooking, hiking, playing competitive kickball, and paddle boarding on the lake.

I am originally from New York and have lived in Raleigh, NC for the past four years. My background is in mental health counseling, and I have enjoyed working with children and families for over twenty years. I have worked with high-risk populations in various settings, including rural communities, hospitals, state facilities, non-profits, and federal programs. I enjoy the work I do, because it allows me to help others and provide resources and services to so many that need the assistance. We are often the voice to many that go unheard and suffer in silence. We work to provide them with hope, support, and guidance.

Prior to joining Duke, I worked at Dorothea Dix and then practiced as a clinical pharmacist practitioner (CPP) managing lipid and anticoagulation clinics in various Research Triangle locations. Dr. Jackson Carter serves Duke employees and their dependents as senior clinical account executive with Express Scripts. She manages the Duke pharmacy benefit and works with physician leadership on realizing cost savings, improving patient care, and increasing patient safety. Through her efforts at Duke, Dr. Jackson Carter was awarded a "Pharmacy Benefit Management Institute Excellence Award" in 2016 for innovative care management strategies. Dr. Jackson Carter graduated from UNC Chapel Hill with a degree in biology and doctorate in pharmacy. She completed a residency in ambulatory care with Mission Hospitals and holds her ACCP board certification in ambulatory care.

I obtained my pharmacy degree from the South Carolina College of Pharmacy. I also completed a PGY-1 Managed Care and PGY-2 Ambulatory Care residency with Kaiser Permanente of Georgia. Prior to coming to Duke, I worked as a Clinical Pharmacy Specialist at Harris Health System in Houston, TX, managing patients with diabetes, hypertension, CAD, hyperlipidemia, and on anticoagulation. My interests also include disease state management in the primary care setting. I have a strong passion for improving patient outcomes and educating the next generation of pharmacists.

I graduated from the University of New England in Portland, Maine. I completed two years of pharmacy residency training at Novant Health with an ambulatory care focus. Prior to Duke, I worked at Cone Health in Greensboro as an embedded primary care pharmacist, focusing on management of diabetes, hypertension, hyperlipidemia, asthma/COPD, osteoporosis, and other chronic diseases. I really enjoy building relationships with patients and working collaboratively with the healthcare team to develop the best possible care.

I obtained my pharmacy degree from the UNC Eshelman School of Pharmacy, as well as a PGY-1 with a focus in Ambulatory Care, and I am a Board Certified Pharmacist in Ambulatory Care. Prior to coming to Duke, I worked as a Clinical Pharmacist at Advocate Medical Group in Chicago, Illinois, managing patients with diabetes, hypertension, heart failure, hyperlipidemia, and on anticoagulation. I also worked as an Assistant Professor at Chicago State University. I am interested in chronic disease states and drug development research. Being a chronic disease state pharmacist has allowed me to help and educate patients regarding preventative medicine. It also has given me the opportunity to educate and mold young pharmacists regarding our profession's role.

I graduated from the UNC Eshelman School of Pharmacy with my Doctor of Pharmacy. I completed PGY1 and PGY2 pharmacy residencies in Ambulatory Care at Virginia Commonwealth University Health and The Ohio State University Wexner Medical Center, respectively. I am a Board Certified Ambulatory Care Pharmacist (BCACP), and I serve as an Adjunct Assistant Professor of Clinical Education with UNC Eshelman School of Pharmacy. Prior to Duke, I worked with WakeMed Primary Care as a Clinical Pharmacist Practitioner (CPP). I am passionate about improving health outcomes by optimizing medications used to treat diabetes, hypertension, and hyperlipidemia to prevent atherosclerotic cardiovascular disease and chronic kidney disease.

I am a Certified Pharmacy Technician currently enrolled in Durham Technical Community College pursuing a degree in nursing. I have been in pharmacy since 2004, where I began working in a retail pharmacy as a cashier. An opportunity presented itself when I received the chance to move into a tech position. I became determined to succeed by teaching myself how to read sig codes, brand generic drugs and their uses, and basic pharmacy operations. I quickly recognized the joy associated with successfully working with all backgrounds of people and contributing to the well-being and health of the community. Since then I’ve worked my way throughout various areas or pharmacy including inpatient, IV preparation, chemotherapy, medication administration, and some work with drug efficacy and safety. I am excited about the patient centered work that is being done through DukeWELL and PHMO, and I look forward to contributing to the success of our patients.

"I am a certified pharmacy technician with over 12 years of experience in sales and pharmacy. I have worked in a variety of pharmacy settings, ranging from IV chemotherapy preparation and medication procurement to prior authorizations and patient assistance. I attended Appalachian State University, where I graduated with a bachelor's degree in cellular and molecular biology with minor concentrations in chemistry and business. I currently serve DukeWELL patients in Orange, Durham and Alamance Counties."

"I am a certified pharmacy technician who attended Virginia Union University and Chowan University, where I studied allied health sciences with a minor in chemistry.  My retail pharmacy experience began in 2003, when I worked at a local pharmacy and soda shop in my community.  Since then, I have worked in a variety of pharmacy settings, including supporting pediatrics, Duke’s Department of Pharmacy Continuity of Care Program, pharmacy billing, and even working in our U.S. Army Military Treatment Facility pharmacies.  I currently provide home visit support to DukeWELL patients who live in Wake, Durham, Person, Vance and Granville Counties."

I have been a Certified Pharmacy Technician for the past nine years with experience in independent, retail, and outpatient pharmacies. I enjoy the work I do, because I am able to help patients be consistent with their medication regimen, as well as helping with medication affordability, speaking with insurance companies, or overcoming any barriers preventing them from getting the medication they need, which will result in overall better health for the patient. It is a pleasure to know that my work impacts so many patients to achieve better health.
