What can you expect when you work with DukeWELL?
1. We’ll start with a short evaluation to help us understand your unique care needs.
2. You’ll be assigned to a DukeWELL team member who can best support you. We will schedule your first appointment and connect in a way that works best for you. We meet with patients over the phone, on video calls, or in your home.
3. At your first meeting, you and your assigned DukeWELL team member will create a care plan that includes a list of health goals to tackle together. We will make sure you have what you need to succeed.
4. DukeWELL will support you as you work towards your goals. You’ll meet with DukeWELL between office visits with your provider, and we’ll make sure you have access to additional DukeWELL team members or community organizations that might assist you with your health goals.
5. DukeWELL will partner with your provider team. From start to finish, we’ll keep your providers updated on your progress and your care plan. We’ll make sure that the entire care team is focused on being there for you.
